Long-term Condition Coordinator

The Long-Term Condition Coordinator will proactively identify and work with people, including those with long term conditions, to provide organisation and navigation of care and support across health and care services., The primary responsibilities of the Long-Term Condition Coordinator are listed in the job description. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

We work from two sites, Girlington Health Centre and Allerton Health Centre.
