The QI and Innovation role is split between clinical work and dedicated time for QI and innovation (13 days every 8 weeks). The successful applicant will be based on General ICU for 12 months and will be supervised by the directorate QI lead and work closely with the unit specific leads to deliver projects. The role will provide an opportunity to develop the candidates understanding of clinical improvement, leadership, leadership of change, self-development, innovation, supporting and spreading learning.
The post holder is not required to have any previous QI and innovation experience, but this post is aimed at doctors who intend a career in intensive care medicine with an interest in QI and innovation. They should be highly self-motivated and have an interest in the use and development of digital tools and technologies. Fellows will be encouraged to present at meetings and submit work to international meetings with support.
Clinical work will be performed on General ICU for the whole 12-month period as a Junior Clinical Fellow. Clinical fellows (CFs) are required to work as part of the critical care medical and multidisciplinary team. We can accommodate experienced trainees / CFs looking to broaden their experience (including subspecialty specific) and novice trainees / CFs with no prior ICM experience. Duties and responsibilities will be commensurate with the individual candidate's experience but are overseen by consultant intensivists at all times.

Working for our organisation
The St George's Adult Critical Care Directorate conducts a comprehensive teaching programme covering all of the subspecialties and access to wider teaching across the hospital (Trauma/Respiratory/Anaesthetics/Cardiac Surgery). There is a highly active research group, weekly journal club and dedicated echo team within the ICU. The successful applicant will be expected to attend and deliver teaching.
Although the opportunities for quality improvement and innovation are significant, the role retains a commitment to on call and other day to day clinical activities as part of the critical care team. Development of QI interest will take place on flexi days and the standard days falling Monday to Wednesday.
Please note that while every effort will be made to accommodate this work pattern, in exceptional circumstances doctors may be required to perform clinical duties on their flexi and QI days and this may include covering other units when staffing levels dictate.
The Directorate has an outstanding track record as a route to speciality training with 90% of fellows successful at the recent Anaesthetic and Intensive Care interview rounds. Successful candidates are encouraged to seek advice and assistance with any planned speciality applications early., GICU is a 24 bedded unit offering all aspects of level 2 and level 3 critical care (with the current exception of Extracorporeal Circulatory Support) to a wide variety of patients. The workload is currently a broad mix of general medical patients as well as high risk elective and emergency surgical patients and major trauma. GICU admits approximately 1500 patients per year. There is a dedicated, post ICU, follow-up team that provides in-patient review, rehabilitation co-ordination and a weekly, consultant delivered, out-patient clinic.
Under the supervision of more senior colleagues, the successful candidate will be able to develop the necessary skills to see patient referrals and evaluate where, how and when to admit a deteriorating patient and how to stabilise and manage the patient in ICU.
CTICU has 21 beds and offers all aspects of level 2 and level 3 critical care to cardiothoracic surgery, cardiology and vascular surgery patients as well as when the need arises a breadth of general intensive care patients. CTICU has also recently expanded to house the St George's Cardiogenic Shock service. This was launched in 2022 and has developed over the past year to now involve the use of mechanical support devices including IABP, Impella and VA-ECMO. The remit of the Cardiogenic Shock service is to identify patients in low cardiac output states secondary to cardiogenic shock and to rapidly convene an MDT meeting to discuss and agree management options.
Successful candidates will assist with management of all CTICU patients including generic intensive care support, advanced respiratory support, advanced cardiovascular monitoring (including pulmonary artery floatation catheters), intra-aortic balloon pumps, trans-thoracic and trans-oesophageal echocardiography and renal replacement therapy.
NICU is a 18 bed specialist tertiary referral intensive care providing elective and emergency care for neurosurgical, neurology and stroke patients. The patient admission profiles include neurology patients with a variety of diagnoses requiring close monitoring and organ support. The unit also provides level 2 and 3 care to Stroke patients post-thrombectomy and other high risk patients requiring interventional radiology as well as general intensive care patients.
The post holder will assist with the management of patients with severe traumatic brain injury, spinal cord injury, subarachnoid haemorrhage, elective and emergency post-operative neurosurgical cases and patients with neurological disease requiring advanced organ monitoring and support.
