Full Job Description

Role Description and Responsibilities Work alongside all GP practice staff as an integrated part of the team, undertaking numerous medicines related activities. Support reception staff to process repeat prescription ordering. Review requests that come in, ensure that any appropriate monitoring has been done or arrange this, contact patients to understand any issues that arise. Pass to prescriber for clinical review and issuing with appropriate information attached. Medicines reconciliation from discharge letters and out patient appointments to ensure timely and safe changes and address any concerns ahead of issuing by prescribers. Help patients to get the best from their medicines by:- o Dealing with ad-hoc patient and healthcare-related queries regarding medications. o Be involved with Annual Chronic Disease review process as part of GP practice MDT by helping review and discuss any medication queries with patients and support new medication initiation. o Specific clinics to review
medications to help understanding of own medicines, support medication synchronisation, concordance. Help ensure practice is following appropriate prescribing guidelines via audit and review of safety alerts and new guidance. Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing. Take a central role in running and enhancing practices current processes on safe prescribing and monitoring of High Risk Drugs. Take a central role in the clinical aspects of shared care protocols and liaising with specialist pharmacists for more complex patients Promotion of electronic repeat dispensing and online ordering Work with local pharmacies to ensure good relationship for benefit of patients. Support nurse and HCA with monitoring and regulations around storage of medications in surgery. Teach students and GP trainees on how practice prescribing system works and their role within it. Support research projects that are being undertaken within the practice. Work
alongside all PCN Pharmacy staff as an integrated part of the team, undertaking numerous medicines related activities. Support the monitoring of medications in accordance with protocols and medication/patient risk stratification tools Help patients to get the best from their medicines by:- o Liaising with PCN pharmacists with clinical queries o Arranging SMRs both via audit process and ad-hoc for those that need o Work with PCN Pharmacy Technicians to support audit work required for PCN contract o Participate in PCN pharmacy meetings to improve engagement between practice and PCN Work alongside all ICB staff as an integrated part of the team, undertaking numerous medicines related activities. Carry out medicine switches in GP practices in line with practice and ICB agreed specific protocols reviewing patients records; amending records and informing patients Increase awareness of role of pharmacy technician within the practice and to promote issues relevant to prescribing and
medicines optimisation Contribute to minimising clinical risks and aiming to reduce wasted medicines. To undertake, participate and share the outcomes of clinical audits to facilitate improvement and changes in practice Assist the practice in achieving national requirements, NICE implementation and utilisation of medicines optimisation initiatives.
