Full Job Description

· Identify, local, low/no cost solutions to address their needs & aspirations via community resources

· Connect to local community and voluntary groups offering a range of activities and services.

· Develop behaviors to help improve health and wellbeing;

· Identify strengths/capacities of individuals to solve their own problems

· Build valued, supportive relationships and identify opportunities to contribute to their communities

· Improve their general welfare, with a specific focus on reducing loneliness and improving community connectedness

Areas of Specific Responsibility

a) linking individuals to community support services, groups and activities in the Southampton area. This includes access to social activities, befriending services, housing support, finance and debt support services, food provisions and other community services that meet individual social needs. This is achieved via information sharing, direct referrals and telephone support.

b) complete holistic assessments with service users, both in person and over the phone, to identity key support needs, goals and aspirations, and develop a person-centred support plan.

c) Support with duty cover, including duty calls, email enquiries and referrals received via the SO: Linked service.

d) to maintain thorough records of your interventions in line with GDPR principles.

e) work with Integrated Teams and local health services to support their work within the target groups. Focus on supporting community-based practice and early intervention.

f)

o Level 1 - Contact with service users who are able to plan their needs but need some additional assistance. To provide information and advice and/or limited support (people can reconnect at later date if needed). Advice to include: accessing information, services or other community resources, including information made available on our website SOLID.

o Level 2 - Intermediary level support. A number of appointments are arranged and a plan produced to address service user needs. Achieved through holistic person-centred needs assessment and support planning.

o Level 3- Advanced level support: direct support and guidance over an extended period. One-to-one support for people wanting to build relationships; achieve self-sufficiency or find practical solutions to problems. Achieved thorough holistic person-centred needs assessment and support planning.

NB: Target Groups are defined as:

1. People who are frequent attenders to primary care and or urgent care services

2. People who are not receiving intensive case management from a health or social care professional, but have an identified significant underlying unmet need.

NB: Expected outcomes for the project are identified as:

1. To empower people to take control/action of their health and wellbeing

2. To build and maximise community opportunities

3. To reduce pressure on health and social care services

4. To challenge services to become person centred

5. To help people who can't help themselves

6. Partnership approach - making a difference

7. Identifying gaps and building on what we already have

· Community groups, facilities and local services within Southampton

· Equal Opportunities issues

· Personalisation and public health agendas, including a holistic view of health and social care service provision

· Knowledge of SPECTRUM's Community Navigator project and expected outcomes.

· Knowledge of SPECTRUM's Business Plan and activities: ensure your role co-ordinates and communicates efficiently and adds value to the work of colleagues

· Independent Living / promoting individual choice and control

Skills in:

· IT Skills: word processing, spreadsheets, Internet, web based searches, social media and databases

· Excellent communication skills (both verbal and written), able to build professional relationships with people from wide range of backgrounds

· Competent with words and numbers, able to write reports

· Time Management: Prioritise your workload and tasks to enable your Job Description to be met.

· Attitudes and manner:Candidates will need to exhibit an enthusiasm to promote Independent Living and the empowerment of people. Demonstrate readiness to acquire new skills and challenge your own attitudes.

· Presentation skills

· Supportive nature whilst able to set and maintain clear boundaries

Experience of:

· At least 6 months experience of working in health, social care or information and advice services that are in direct contact with service users (paid or voluntary)

· Experience of person-centred planning

· Able to develop and maintain productive relationships with health, social care and community based professionals so that they feel able to refer users to the service

· Personal experience of living in Southampton

· Working paid or unpaid within a rights or equality based organisation

· Supporting people to live independently

No formal qualifications are required. This post is subject to a criminal DBS check.
