Full Job Description

We have an exciting opportunity to join our Community Stroke Service Team as a Band 7 Team Lead/Advanced Practitioner AHP/Nurse, to work across the Scarborough and Ryedale locality.
You will be expected to lead and support the provision of a seamless Stroke service from the local acute hospitals into the community. The team provides assessment and rehabilitation within the community setting for adults, from 18 years upwards who have had a Stroke. We work closely with professionals, services and agencies across the whole Stroke pathway.
You would be part of a small, dedicated, knowledgeable multi-disciplinary stroke team. You will be joining the team at an exciting time of service development towards implementing the National Service Model for an Integrated Community Stroke Service, with a focus on creating a robust early supported stroke discharge service., You will be responsible with the Service Manager and Clinical Lead/Matron for the provision and development of the Community Stroke Service across Scarborough and Ryedale. You will work within a multidisciplinary team, including Speech and Language Therapists, Occupational Therapists, Physiotherapists and support staff and have line management and supervisory responsibilities for junior staff within the stroke team.
You will hold a caseload of patients and be accountable in your own area of clinical responsibility, ensuring the delivery of evidence based clinical interventions and practice, This post will be subject to successful completion of a 6 month probationary period where applicable, further details would be included within any offer letter.
Where operationally feasible the Trust supports agile working, there is an expectation of attendance of 1 day per week at a Trust site as a minimum operationally. Unless specifically stated in the advert that a role will have home as the base of work, all roles will have the option to work from a Trust base if remote working is not for you.
If required for the post the ability to commute within the Trusts geographical area with access to the appropriate means of transport would be essential.

Applicants should have a strong value base and promote holistic person-centred care. Previous experience of working within Stroke services and community experience would be advantageous. Additional specific training can be provided.
The post holder needs to be flexible, enthusiastic, have good time management skills, confident IT skills, and work well in a team as well as independently.
