Any applications via agency will also not be accepted., This is an exciting opportunity for a registered general nurse or qualified physiotherapist or occupational therapist to work within the Transitional Care and Flow team within the umbrella of the Urgent and Emergency care division.
The Transitional care practitioners are pivotal in supporting system flow within the community. The transitional care practitioner's role consists of assisting with the overseeing of the patients within our community Transitional Care (TC) and flow beds situated in specific nursing and care homes county wide.
This is a role that allows you to develop your case load management and have a broader oversite of the care given to a multitude of patients who are medically fit but require a level of rehabilitation with the aim to safely discharge home.

Although our service's main focus is on preventative, personalised care is key: we work to promote the principles of 'Home First', acknowledging that, for most of our patients, engaging in meaningful activities in their home environment is the best way to promote self-care and a sense of well-being, however some need physical therapy to ensure that they are able to safely return home and our service provides this in our community Transitional Care beds.
Our team is embedded within the Transitional care service and work alongside the Assertive In- Reach within the integrated discharge hub along with the therapists completing the therapies in the community beds and the nurses with overview of the nursing and care homes in Transitional care and flow.
The practitioners work alongside key stakeholders to facilitate appropriate care and flow within the TC beds. The ideal candidate will demonstrate a passion for excellent quality, person-centred care,with other key partner organisations in order to achieve the best possible outcomes for our patients. Collaborative working, making decisions based upon sound clinical reasoning and having the ability to adapt and flex your way of thinking and approach is essential.

Relevant experience in a health setting with at least two years experience in a relevant setting. Must be a Registered General Nurse or a qualified registered AHP therapist. Able to work alone and as part of a team. Dealing with difficult conversations when required. Assessing patients for further care, working with stakeholders, challenging and reviewing information with stakeholders. Supporting and facilitating system flow. Able to commute across county when required. To be able to work flexibly to accommodate the service. Ability to problem solve.
